How to Refer

We’ve streamlined our process to make it as straightforward and stress-free as possible for all involved. Here’s how it works:

Contact our admissions team to discuss the individual’s history, current situation, and care requirements. We’re happy to have an initial conversation to explore suitability and answer any questions you may have.

Step 2: Assessment & Care Planning
If a placement is suitable, we carry out a comprehensive assessment—considering mental health history, physical health, risk factors, and care preferences. This helps us develop a personalised care plan that meets the individual’s unique needs.

Step 3: Collaborative Transition
Working closely with NHS professionals, Social Services, families, and other stakeholders, we coordinate a tailored transition plan into one of our homes. We ensure the move is smooth, considered, and as comfortable as possible for the individual.
